Facility Request

Below are the steps to schedule the use of the facilities at LCC.  If you have any questions you can email us at calendar (at) lifecommunity.com.  

INSTRUCTIONS:

1.  Complete our online schedule request form.

2.  Download and read "Buidling Use Request Guidelines and Fees"

2a.  If you're requesting to use our children's areas please download and read "Children's Area Use Additional Guidelines

3.  Download, sign and return the "Building Use Request - Agreement Form"
